I think FlexiPanels is somewhat hard to find with Dreamweaver CS4. I suggest to create a menu item under the "Insert" top menu.

I have installed the FlexiPanels but cannot find it under the install tab. Everything appears fine in the extension manager

Hello,
Well, looks like Guy is right, we have to make our extension more visible.
You can find the extension buttons on the insert panel. To open that panel go to "Window->Insert" from the top menu.
On the insert panel there are several categories, you have to select FlexiPanels category and you will see the buttons.
