Problem solved...
By default the "includes" folder was in the wrong place. I moved it to the public _html folder and everything started working.
Looks pretty good now.

One more related issue.
When I update the menu it it continues to write the "includes" folder to the wrong location. I have to cut and paste it to the correct folder to get updates to function.
How do I permanently change the save location of the Includes folder?

Hello,
The extension should save the “includes” folder on the root folder of your website. All the links should point to that location afterwards. On each page, when the menu is inserted the links that are created to the includes folder should be correct. Are you using a template to develop the page?
If you are using a template, the templates are saved by default inside a folder named “templates” from your website. Because of this, the link to the includes folder is created with a “../” in front. When you use that template on a page, that’s on a different folder level than the template, the links are broke.

There are no templates in my website. The extension is writing the includes folder to the directory above the root folder for my website. It is complicated to move the files and keep the links correct and so forth. It is very inconvenient.
Can you change the location the file saves to. If not the software is of little use to me.
The inconvenience of moving the files then fixing the links and dealing the involuntary automatic uploading the files to the wrong location on the server. I end up having to keep two copies of the "includes" locally and on the server. If I delete the one outside of the directory then I can't edit the menu because the extension does not see the menu once I move the includes. the juggling process is leading to errors in the links when ever I attempt to edit.
Am I the first to have this problem? I use Ipower as my host and they are a very large webhosting company. Their is one tier above the "root" in Ipower's file system and no way around this according to Ipower. So I have to ask once more is there a way to change the save location of your software? If not just say so... I don't want to waste much more time on your software if it won't work properly with Ipower file system.

Hello,
The extension it’s not creating the folder in the wrong place. It creates the includes folder in the folder set as root for the Dreamweaver Site.
